2020-2021 Required KSHSAA Physical Forms

 

We know this has been a crazy spring for all!  The district is still making plans for the 2020-21 school year.  KSHSAA has released the NEW KSHSAA Physical Forms for the 2020-21 school year.  All student athletes in grades 7-12 for the 20-21 school term that will be participating in any extra-curricular sport (including cheerleading/dance) are required to have a current KSHSAA physical examination before the first day of practice. The first day of fall practice for 20-21 is August 17, 2020.  The physical examination must have been given after May 1, 2020 in order to be valid for the following school year.  The appropriate physical forms are linked below and also available at the Troy Central Office (Mon.-Thurs. - 8 a.m. - 4 p.m.)  This requirement is a part of our compliance with Kansas State High School Activities Association.
